Shortly after she becomes a Gennin (before the mission to Wave Country), Sakura's parents inform her that, many years ago, before the Kyuubi came to Hidden Leaf, they arranged a marriage for her with a powerful clan. The last son of that clan is on her gennin team. (Cue Inner Sakura erupting in joy over being engaged to Sasuke.) The clan is Uzumaki. (Cue Sakura crashing into shock and despair.) Then they, or perhaps the Hokage, inform her that he's the vessel that the Kyuubi was sealed in, and as his future wife, it is her responsibility to make sure it never gets out. Perhaps the Hokage suggests that she should be kind ot him, so that he never wants to let it out, or maybe she figures it out on her own. Either way, it should trigger a serious explosion on the subject of the way the villagers have treated him.
How would the village react when the engagement was announced (other than various Bishounen fangirls thinking 'One less rival' or 'better her than me')? Would Sakura be shunned, or pitied?
How different would Naruto become, with Sakura being nice to him, and perhaps actively helping him grow stronger and more skilled? Will Kyuubi have an opinion?
How will this change Sakura?
Would Sasuke be any different, at all?
What about poor Hinata? Does the concept of 'official mistress' exist in Konoha? If it does, would Hinata have the nerve to try for it? Would Sakura be even slightly receptive to the idea?
