To keep this short and sweet I was thinking about how Fred and George never saw Peter Petegrew's name on the map while they used it, and I think that I figured it out.
As Wormtail was one of the Marauders, he probably knew the inner workings of how it worked and would thus be able to confuse it. Being a pet of Percy would allow him to be near the twins and he might have overheard them talking about a map. All he would have to do then is wait until there was no one around him and cast a spell.
I know fannon states that Peter was only riding on the coattails of the other three, but there is nothing in cannon to state that. It only says that he was looking to be on the winning side.
As to why he appeared on the map in Harry's third year is simple. Sirius Black escapes Azkaban and when Peter finds out is terrified. Peter knows that Sirius will hunt him down for betrayal and in his panic forgets to cast what I'm calling in my head, the hiding spell.
