Well if you use time travel so Keitaro screws a younger version of his grandma it wouldn't be that bad.

If you really want to go for twisted you could have him go back and knock up his grandmaw and then when he gets back from the past he's killed by a Naru punch.

That could make for a good 'irony' fic. Sometimes Fate is indeed a fickle mistress, to use a politically correct expression (the politically incorrect one is 'bitch', of course).

EDIT: granted, the epilogue would be shit, since it would probably focus on the younger Hina, and the funeral of Keitaro.
