Well... the two main Type-Moon translation projects have been mentioned, but I'll list everything that's been translated (or is being translated) currently:

To my knowledge, that's everything. I think there was talk of Act Cadenza Version B getting translated, but I haven't seen any posts about its progress.
On Plus-Disc/Plus+Disc, the difference between the two is that Plus+Disc is a later version, and thus has more content. They're also in slightly different formats... Oh, Plus-Disc runs off the free demo you can download from the official Type-Moon website whereas Plus+Disc runs off the version from Tsukibako (The Tsukihime, Plus+Disc and Kagetsu Tohya pack).
