I'm not sure there'd be a Suigetsu in my version.

He always felt like an unneccesary member of Sasuke's team. Juugo and Karin both had skills that made them useful for tracking down Itachi which was the reason the team was put together while Suigetsu feels like some extra muscle they never really needed. He had his own ambitions that had nothing to do with Sasuke and really should have just gone back to Kiri yet went along him anyway and basically failed at everything he tried to achieve.

If I wanted him with Sasuke, I'd play around with his abilities and history some. His water body would be a kekkei genkai rather than a hiden ninjutsu and he wouldn't have been a shinobi of Kiri but an exile of the Land of Water. He's interested in collecting the seven swords but for his personal interests, not because he plans to be one of the Seven Ninja Swordsmen. So, like Karin and Juugo, he has no real attachments outside of the team.

But I'd prefer him not to be with Sasuke at all. Kabuto feels like a much better fit for me, far more useful to his goals and would provide an actual medic instead of randomly throwing in Karin's unexplained healing power later in the game.

Instead Suigetsu would be a loyal shinobi of the Hidden Mist and one of the Seven Ninja Swordsmen. If nothing else, it bumps up the number of relevant characters from that village when they start showing up from the Kage Summit onwards.

Yeah I can buy that. I do like Suigetsu's powers and personality, but you are right that Sasuke didn't really need his help (well he did when they were fighting Killer B, but Sasuke wasn't planning to join Akatsuki that early on) and he honestly had no reason to tag along with Sasuke aside from maybe hoping that finding Itachi would also lead him to Kisame so he could take his sword (which didn't work out for him).

Perhaps Sasuke could still free Suigetsu from imprisonment like in canon, but he doesn't ask Suigetsu to join him. Suigetsu thanks him and returns to Kirigakure (and makes a detour to retrieve Zabuza's sword along the way). Perhaps they meet again later during the war, or Suigetsu even takes Choujuurou's place as Mei Terumi's bodyguard at the Kage Summit.

Sasuke freeing a few people who actually show up again here and there would be cool. Everyone he did that for in canon just plain vanished without even a little mention of them spreading the word like Suigetsu told them to.

Expanding on an idea I mentioned for Sai and Yamato:

Sai would be one of the war orphans taken by Root and used as a test subject for Danzo's experiment with Wood Release. These experiments took place after the Uchiha Massacre when Root had been officially disbanded and was forced to go underground. Danzo revived older experiments on recreating the Wood Release to create a secret weapon.

Out of sixty test subjects, Sai was the only child to survive and was able to use a weakened version of the First Hokage's Wood Release. He was recruited into Root proper to serve as Danzo's apprentice to become the organization’s best agent.

He didn't participate in the Konoha Crush despite his unique abilities because his powers were judged to be too immature to deal with a Tailed Beast. And Danzo had his own reasons for not providing aid there.

Sai is assigned to Team Kakashi for the Tenchi Bridge Reconnaissance Mission under Kakashi’s command – who had been personally treated by Tsunade to help him recover in time for the mission - with the secret objective to spy on Naruto and eliminate Sasuke.
