Since no one seems inclined to try any of the titles I suggested...

Yes, Shirou didn't get rescued in quite the same manner as he did in canon. Gil didn't quite stick around. Instead, the latter almost died, and Kiritsugu used his fading mana/prana to give a fatally injured Shirou the kickstart he needed to live through the mess (Avalon wasn't working quickly enough to keep him alive).
No, it's not. You mean Memento Mori. Momento Mori would be closer to 'moment of death' or something. Eh. Dunno.
Anyway ... for the unmangled Memento Mori version:
Tohno Shiki nee Nanaya Shiki was still severely injured. He still acquired the Mystic Eyes. He still freaked people out until he met Aoko.
Who did not give him a cheapass 'grace period' before his body finally gave out and up.
Instead, she treated him to some soba ... or so he'd thought.
The musings of an immortal Shiki, contemplating the world, the lines, and particularly the insanely complicated lattice of points that constantly shift across his body, in and out of existence, as the Kessen Chu wander his blood-stream.
Possibly interrupted by a flying glomp and wild monkey sex with Arcueid and Ciel.
For the mangled Momento Mori version:
A single moment in the lives of the Shinigami of Soul Society and their allies in the Mortal World.

For the first time since their inception, the Nine Bijuu are all housed into Jinchuuriki.
But something has not happened that they did not expect. Across oceans, across mountains, across fields, and across deserts......the Bijuu begin to.......resonate.
It's a race against time to find out what exactly this resonation means before it destroys the very life fabric of the land. The seals are weakening. The Jinchuuriki are teetering on the edge of sanity. And it could mean the end of everything.
