Usagi Davion, grand-daughter of Hanse Davion's first Champion. Ami Kurita, great-niece of Takashi Kurita. Rei Liao, Kali's twin sister. Makoto Marik, daughter of Duggan Marik. Minako Steiner, long lost daughter of Fredrick Steiner. Setsuna Hakondottir, younger sister of the Elected Prince. Hotaru Calderon, daughter of the lost Edward Calderon. Haruka Avellar, daredevil pilot from the Periphery. Michiru Centralla, who was visiting the local violin conservatory and got drafted as an ambassador.
These nine young women are being trained by Natasha Kerensky's pet surat to defeat the dark forces of Beryl Showers, the evil power behind the Clans and their Dark Khans. When the stars are right, the Great Father will awaken and shroud all the worlds in darkness unless the Senshi and their mysterious ally Endymion Focht can stop her.

Basic Idea: The Senshi as viewed through the eyes of some dimensional spanning nation that has a interest in the Earth and for whatever reason that has never encountered something like the Senshi in any other reality.
First from as some backwater magic users or girls that happen to possess a strange technology to the slow realization that they are far more then what they seem to the horrifying knowledge that they are actually ancient 'edritch abominations' led by a 'terrifying cosmic horror'...
Not that Usagi or any the other Senshi would agree with said sentiment or their families for that matter ...repeatedly needing to change the diapers of somebody when they're a baby or having to wake them up everyday to get them to school everyday makes it rather hard to view as somebody as a horror from beyond the stars.
However a bit of miscommunication, lots and lots of paranoia, the possibility of what the Senshi could do if they wanted, and panicked assumptions can make nearly anything seem somewhat possible.

Ami Diamond was the daughter of Kula and K'. Her ice powers were matched by her cool intellect.
Rei Kusanagi was her father's heir, with just as fiery a temper as her father, Kyo...and the Magatama to match.

After Mami gets her head bitten off, Madoka, who is even canonically a bit fangirlish, promptly wishes for Sailor Moon to rescue them. Given how ludicrously powerful she is, this actually succeeds, and she turns a fictional character real. One Moon Healing Escalation later, and Charlotte is no longer a witch, and Mami is no longer dead. Hilarity ensues.
I was just rereading the Anvil Galaxy sourcebook for Rifts and noticed that description of the Cosmic Forge bares a uncanny resemblance to that of the Galaxy Cauldron seems to work. So what if they where the same thing or similarly created objects of the Firsts and when Usagi finally exits after healing the damage caused by Chaos she finds herself not in the heart of Sagittarius Zero Star but in the Three Galaxies?
Of course to make things worse Usagi's presence doesn't go entirely unnoticed. Some dream of a figure emerging from the light of the Forge, some are merely interested in the newest player on block, and a very few might be close to realizing the truth.
Now she must find a way home without setting off a apocalyptic war within the Three Galaxies and deal with a number of factions that are uncomfortably interested in what seems to a unknown goddess.
However no matter how much she might want to return home Usagi finds it impossible to keep from trying to interfere just a bit...

While the cats were in stasis, Galaxia destroyed Mau, but before she could the planet's Sailor Crystal. Sailor Mau sent it her power far, far away. When Luna is unsealed, she notices something, quite not right about her. For one thing, she's naked and another thing she's no longer a cat. Luna, for once can finally help her charges and fight the scum that destroyed the kingdom she once fought so hard to protect but because of her powers she can no longer reliably go back to her form. Without her cat form to protect her. she has to rely on Usagi and the senshi to shelter and protect her from a world she doesn't quite understand. Which gives her a much better understanding as to why Usagi want the sense of normalcy that she wants so badly. Sailor Luna's existence would also probably keep Mamoru back a lot. A extra senshi equals less chance for him to swoop in and save the day.
Frankly, I would just love to see Luna confronting Beryl once and for all for what she did to Queen Serenity with Venus complaining about Kill steals.

I would assume it'd punch a hole in space-time to a universe where Sailor Moon is real, and then suck her through it. Maybe give her a link to the local version of the moon to draw power from, though that might not be necessary. Madoka's easily powerful enough to do that, and it's probably the most direct way to grant her wish of having Sailor Moon appear to save them. As for Kyuubey's plan, I'd assume in the short term it'd involve just milking her for extra magical girl/witch transformations (as the witches she turns back into magical girls wind up turning back into witches), and maybe of a long-term goal of trying to get to the Sailor Moon universe, and then finding and exploiting the Galaxy Cauldron to end entropy. Or getting Usagi to create a new one, so they can do the same.
You mean the Coalition States? Outside of a few very unlucky rifted soldiers their pretty much contained in the remains of what was once the US.
Besides sending Usagi to Rifts Earth without any knowledge of it would probably be a really bad bad thing to do. She isn't the type to simply stand around as innocents die gruesomely agonizing deaths and entire so-called balance of power is one massive powder-keg that she's likely to accidentally set it off in a megaverse wide shitstorm.

Shingo sighed. Usagi was just too careless with Chibichibi. She honestly called herself a big sister? Who let's a two year old play with a heavy glass incense pot out of all things.

After the debacle that was the Majin Buu arc, Vegeta decides that if he wants to get stronger than Goku, he needs to get into worthwhile fights that Goku isn't taking part in. Since this is impossible on Earth, and his wife is capable of building a time machine that creates alternate universes, he goes and talks to her about the possibilities. So, she goes and builds him a machine that will transport him to alternate timelines, so he can get into worthwhile fights and get stronger.
The first timeline he tries is the manga version of Sailor Moon, possibly right as the Galaxia arc is starting. Hilarity ensues.

The mere idea of a Dragon Ball Z/Sailor Moon crossover is no enough.
In any case, I was randomly thinking about Nemesis, which was probably a mixture both of the Nemesis darkstar theory and Planet X that was proposed to be beyond Pluto before we started discovering a raft of trans-Neptunian objects in the late 90s (some were discovered earlier, but I would generally count as a 'planet' as any body large enough to have a spherical shape due to the action of its gravity acting inward on itself, though that's not the IAU definition).
That got me to thinking about the historical idea of the 'fifth planet', between Mars and Jupiter, which would have been destroyed, splintering apart into the asteroid belt. What caught my attention was that the popular name attributed to this fifth planet is Phaeton, the son of Helios in Greek mythology.
So, yeah, I dunno idea-wise. Maybe have a planet there during Silver Millenium-times that was destroyed either by the youma or perhaps even earlier by a Death Reborn Revolution? Or maybe in the future time if Chibi and one of her bitches (Helios) actually have a sexual liason they could attempt to seed the creation of a new world in the asteroid belt. Which would fit insofar as Chibi could then have a throne planet inside the asteroid belt where her other four loyal bitches derive their planetary powers from.

After my Minako/Artemis topic fell to the wayside, I put the idea on the back-burner for awhile, but for some reason it transformed on me.
Luna completely and utterly fails to find any of the other Scouts, so Minako has to take on the brunt of defeating all the youma grunts by herself. Over time, this begins to wear down on her psyche, and though she accomplishes killing Beryl with the Holy Blade, she sacrifices her life to defeat Metaria pre-emptively before it has a chance to cause any irreversible damage. Maybe she gains her Venus Crystal prematurely from Artemis but isn't able to properly control the power to save her from the backlash? Or perhaps in drawing the Holy Blade, the remainder of Queen Serenity on the moon grants her some special-one time power.
The other possibility would just be complete crippling of all her Senshi powers and she basically retires. No post Dark-Kingdom shenanigans occurring in this timeline because then Earth would be fucked.
So yeah, a real tragedy type story.

I was reading an old Ranma/Sailor moon cross where the timefuckery of the Black moon clan changed a bit o recent history leading to a world where the senshi acted like Puella magi do (not the witch thing just the fact that it's serious business and they hate each other naturally). These senshi of course were obliterated and replace by the canon ones of course to keep all the misunderstandings going.
Anyway the idea i had was to explore what these senshi could have been like. The main divergence was Luna recognizing Usagi as the princess and keeping her away from Naru's. This lead to Naru dying and Usagi realizing shit got real and getting serious. I just want to see more on this concept.
